APPCARE model for HOSPITAL CARE
With regard to hospital care management of frail elderly patients, the main challenges perceived are all related to the lack of a standardized and appropriate assessment of the status of the elderly patients when admitted to hospital. Usually, complex elderly patients are assisted as if they just have a unique pathology and complex situations (e.g. multimorbidity) are not taking into account. Professionals prioritize the diagnosis and care based on the seriousness of complaints, rather than providing comprehensive care.
To meet these challenges the APPCARE model includes:
- Standardized application of the Comprehensive Geriatric Assessment (including pre-morbid Barthel Index – health status 2 week before the acute event), within 48 hour after the hospital admittance;
- Assessment of the social and environmental context of the patient (in particular, the living condition – alone, home-assisted by relatives or informal caregivers, homecare assistance with formal caregivers, nursing home);
- 48h intensive care (short term observation period) under the responsibility of the geriatric team in charge of diagnosis and clinical patient;
- Standardized communication flow for a timely exchange of patient’s information from hospital to territorial care.
